Jonathan Dychko, MD, Founder

Jon was raised in a town outside Orlando, Florida, called Winter Springs. At Winter Springs High School, Jon met Liz, and they became high school sweethearts. They were married in 2011 when their journey into medicine began. After graduating from the University of Central Florida, Jon and Liz moved out of the country to Grenada. It was there that Jon started his medical school training at St. George’s University. After spending almost two years in Grenada, Jon and Liz returned to the States to finish Jon’s clinical rotations in Atlanta, Georgia. Jon graduated from Medical school in the summer of 2019 and was matched at Brandon Regional Hospital in Brandon, Florida. Having been from Florida, Jon and Liz have desired to practice in the Sunshine State. During Jon’s residency, he learned how the healthcare system operates. He also knew where the overwhelming need was located. Many young and talented doctors have pursued procedure-based specialties, and over the last 10 years, this has grown to create a growing need in the primary care space. At Brandon, Jon learned the first question asked to any patient coming into the hospital: “Who is your primary care doctor”? It was that question, and after much consideration, that Jon learned how critical the role was to have an excellent Primary physician who understands the healthcare system. When asked about his decision, Jon said, “It is critical to educate the patient and the patient’s family, all the while helping them navigate the ever-growing complexity of healthcare today.” There is a need everywhere for good communication and a Primary Internal Medicine doctor to bring value to the patient experience”. During his time at residency, Jon had the privilege of serving the community members of Sun City Center, and after having many requests to come and serve the community, Jon, with the help of many partnerships in the area, decided to open a private independent practice in Sun City Center. “In a healthcare world filled with huge corporations, I believe there is a need for a practice to focus on value for the patient, relationships with the patient, hospitality towards the patient,” Jon says. He focuses on getting into the community, being known by the community, and keeping them in the community. “It will be my life’s work,” and with the opportunity before him, he humbly says, “I am ready to serve.” And so, Healthy Home Primary Care was created, and it is with joy and eager expectation to say to all his future patients, “Welcome Home.”

Nicole Fore, PA-C

Driven by her passion for serving others, Nicole earned a Bachelor's degree in Natural Science-Health Sciences, followed by a Master of Science in Physician Assistant Studies from Daemen University in Buffalo, New York, in 2018. During her studies, she achieved honors on the Dean’s List and received the Presidential Scholarship. Nicole is a fellow of the American Academy of Physician Assistants and the Florida Academy of Physician Assistants, and she maintains state licensures in both New York and Florida. She has also volunteered for healthcare services globally in the Dominican Republic and Morocco.

Nicole has previous work experience in private practice internal medicine and relevant clinical experience in psychiatry, gastroenterology, and women’s health. She is committed to improving her patient's quality of life through shared decision-making and a focus on preventive healthcare.

Cailin Chapman, PA-C, RDN, LDN, CLT

With a medical education rooted in nutrition, Cailin is a registered dietitian and physician associate emphasizing optimal health based on root cause medicine. She earned a Bachelor of Science in Dietetics from Kansas State University, graduating magna cum laude and further expanding her knowledge and skills by graduating with high honors from Gannon Universities Master of Science in Physician Assistant Studies. 

Cailin specializes in Integrative and Functional Nutrition and Medicine. Additionally, she has completed training in Functional Medicine through the Institute of Functional Medicine and a post-graduate certification in Evidence-based Optimal Nutrition through Harvard T.H. Chan School of Public Health. She is also a Certified Leap Therapist (CLT), an evidence-based protocol on Lifestyle, Eating, and Performance (LEAP), identifying health conditions triggered by adverse food reactions. LEAP utilizes a clinically-based elimination diet to improve health and wellness. 

As an entrepreneur and designer, she started Remnu LLC, a therapeutic pillow company. "My mission with REMNU is to improve your health through enhanced quality and quantity of sleep. This pillow is ideal for back or side sleepers to improve alignment and provide optimal support for a good night's sleep. Quality sleep is imperative for optimal health; a lack of good or great sleep is the cause of so many issues in our daily lives." 

Rugile Nichols, DO

Rugile Nichols, DO, is a primary care physician with a profound love for family and preventive medicine. Her passion for family medicine stems from the opportunity it provides to form enduring relationships with her patients while addressing a wide range of conditions. This love for her field reassures you of her commitment and competence as your primary care physician.

Dr. Nichols' journey to becoming a primary care physician was marked by a commitment to individualized care. She earned a Bachelor of Arts in Psychology from the University of Illinois in Chicago, a Master of Arts in Biomedical Sciences from Midwestern University in Downers Grove, Illinois, and her medical degree from the New York Institute of Technology College of Osteopathic Medicine in 2020. Her training in family and preventive medicine at the University of Arkansas for Medical Sciences further solidified her dedication to providing each patient with comprehensive, personalized care, with a focus on wellness and prevention.

Outside the clinic, Dr. Nichols enjoys spending time with her husband, family, friends, and her three adorable cats. She also loves traveling, going to the beach, rollerblading, hiking, trying new restaurants, and cooking new recipes at home. Although she and her husband grew up in Illinois, they have spent the last few years making Florida their home.

Krista Longmore, FNP-BC

Krista's affiliation with healthcare began at South University, Tampa, where she received an Associate's Degree in Health Science in 2012, graduating with summa cum laude academic honors. In 2014, she furthered her education at the University of South Florida, where she obtained a Bachelor’s Degree in Nursing and was invited to the National Nursing Honor Society, Sigma Theta Tau.

Krista’s first step in the nursing profession was in medical and surgical oncology. She then spent four years in critical care nursing, starting in the ICU and later transitioning to ERs, where she was promoted to the Charge Nurse position. Her leadership skills and medical knowledge were refined as she trained and supervised other nurses in the provision of quality care to patients.

Eager to continue her advancement in medicine, she worked toward completing her Master of Science in Nursing as a Family Nurse Practitioner, all while continuing to work at the hospital. After completing her studies in 2022, she went on to receive board certifications from both the American Association of Nurse Practitioners as well as the American Nurses Credentialing Center. Her background in critical care shaped Krista into a provider with a heart to advocate for all problems as serious as her own, showing empathy, understanding, and a genuine desire to improve lives.

Krista currently contributes to further Healthy Home Primary Care’s efforts to bring high-quality patient care to rehabilitation facilities and nursing homes in Sun City Center.
